A Physiological Soft Tissue Procedure for Hallux Valgus Deformity Correction

چکیده

Hallux valgus is a soft tissue condition of normal bones deviating from their normal alignment due to failure of their stabilizing ligaments. Affected bones are victims rather than culprits. Osteotomy of normal bones to compensate a soft tissue problem can compromise biomechanics of the all-important first ray for function and complicate the matter. To patients, hallux valgus deformity is mainly a function problem of the foot that can eventually produce secondary problems like clawed toes, collapsed arches, and metatarsalgia and bunionette pain. To surgeons, hallux valgus should always be a function issue and their chosen surgical technique should have been proven capable of improving and restoring foot function. A thought provoking case is presented to illustrate why a non-osteotomy and nonarthrodesis soft tissue procedure can correct not only a severe hallux valgus foot but also salvage a failed osteotomy procedure.

Phalangeal Osteotomy for Hallux Valgus

Adequate correction of hallux valgus may be attempted at various levels of the first ray complex, depending on appropriate determination of the apex of the deformity. Methods of correction may include first metatarsal base osteotomy and first metatarsal head osteotomy as well as uncomplicated first metatarsophalangeal joint soft tissue realignment. In those instances in which the major deformit...

Salvaging of a failed Lapidus procedure by the soft tissue osteodesis procedure: A six-year follow-up case report.

Soft tissue approach to hallux valgus deformity correction has not been popular due to concerns of its effectiveness. Osteodesis procedure is a soft tissue technique that has been reported with favorable results, especially its long term deformity recurrence rate was very low. Recurrence complication after Lapidus procedure has seldom been reported and its remedial surgery not much discussed ye...

Proximal metatarsal osteotomy and distal soft tissue reconstruction as treatment for hallux valgus deformity.

The correction of a hallux valgus deformity with a proximal 1(st) metatarsal osteotomy and distal soft tissue repair is achieved with a three-incision technique. A lateral soft tissue release and reconstruction at the first metatarsophalangeal joint releases contracted structures including the lateral capsule, the transverse inter-metatarsal ligament and the conjoint adductor tendon. Rarely is ...

Bunion Surgery: Should You Perform A Lateral Release?

McBride popularized the lateral release in the earlier part of the century as an isolated soft tissue procedure to correct hallux valgus. Since the procedure did not correct the structural deformity, surgeons performed the procedure with fibular sesamoidectomy and/or adductor transfer in an attempt to maximize the soft tissue correction. However, it became apparent that overzealous work within ...

Hallux abducto valgus, commonly referred to as a bunion, may have two components: a rigid or structural component and a dynamic or flexible one. One component usually predominates over the other. The dynamic or flexible component is reducible clinically or after lateral soft-tissue release...
